Kitsap Applied Technologies  Volunteer Application Form - Please fill out the application form and sign it.  

Kitsap Applied Technologies Applicant Background Check Form - Fill out the background check form as well.   Place both forms in an envelope, address it to:  Attention KAT Director, along with the address listed below.  Mail the envelope to KAT or drop it off at the office.
